RS Form W-9 is used to provide taxpayer information, most often when you’re a freelancer or independent worker earning over $600, or when a financial institution, especially overseas, needs your US Social Security Number for reporting purposes. Independent workers, freelancers, and other workers who have earned at least $600.
You can request a copy of the W-9 Form from your employer or download it directly from the IRS website.
How to File the W-9 Form
- Line 1: Enter your legal name, as shown on your tax return or social security card. If you have requested an ITIN, input the name from your W-7.
- Line 2: If you don’t have a business name, leave this blank.
- Line 3a: Check the appropriate box for your business's legal structure so the IRS knows how to classify your information for tax purposes.
- Line 4: This section doesn’t apply if you complete this Form as an individual. It is for Businesses or organizations that qualify for an exemption from backup withholding or FATCA reporting. They must enter the correct exemption code, which is found in the W9 instructions.
- Lines 5-6: Input your mailing address for IRS correspondence.
- Part I Tax Identification Number (TIN):
- Enter your Employer Identification Number (EIN) or Social Security Number (SSN), depending on the structure of your business.
- Part II Certification:
- Sign and date the form to ensure that the information you have provided is accurate.
- If you have a backup withholding for failing to report all interest and dividends on your tax return, you must cross out Item 2.
